Existen situaciones bien identificadas que inciden de manera negativa en el bienestar del paciente; una de ellas, conocer parcialmente el desempeño de una vía aérea artificial; de manera similar, el impacto fisiológico en la vía aérea inferior. El presente documento pretende ampliar el horizonte desde una perspectiva práctica, con el objetivo de ayudar en la toma de decisiones referentes al cuidado de la traqueostomía. Entre ellas se incluyen: uso adecuado de dispositivos de humedad, tiempo para instalar y retirar la cánula, manejo de secreciones, entre otras. Sin lugar a dudas, el entendimiento y la aplicación juiciosa de las estrategias aquí contempladas, incidirán en el manejo de los pacientes con traqueostomía.]]></p></abstract>

There are well identified situations which negatively impacts in the patient's well-being. One of them is the incomplete knowledge of the artificial airway performance; similarly the physiology of the lower airway tract. This paper pretend to wide the horizon, from a practical perspective, in order to help in the decision making process related with tracheostomy care. This include humidity, time of insertion and decannulation, secretions management, among other subjects. Undoubtely, the understanding and judicious applications of the topics revised here, will impact in a better care of the tracheostomized patients.]]></p></abstract>

En el Departamento de Neumolog&iacute;a del Hospital Regional de Alta Especialidad de la Pen&iacute;nsula de Yucat&aacute;n (HRAEPY) se realizaron un total de 279 interconsultas por m&uacute;ltiples morbilidades en el per&iacute;odo de agosto de 2013 a agosto de 2014; de ellas, 4.6% correspondieron a pacientes con una traqueostom&iacute;a. Cabe hacer menci&oacute;n que uno de cada tres casos requiri&oacute; broncoscop&iacute;a de urgencia por obstrucci&oacute;n aguda de la c&aacute;nula para retiro de secreciones espesas. Derivado de estos hallazgos, nos planteamos como objetivo generar un documento din&aacute;mico enfocado en el manejo de los pacientes con traqueostom&iacute;a. La comprensi&oacute;n fisiol&oacute;gica de una v&iacute;a a&eacute;rea artificial llevar&aacute;, sin lugar a dudas, a cambiar las conductas vigentes, aspecto que debe impactar en la evoluci&oacute;n y desenlace del padecimiento particularmente modificando la aparici&oacute;n de complicaciones agudas.</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>ANATOM&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2">La tr&aacute;quea es un tubo fibromuscular cuyo tama&ntilde;o oscila entre 10 y 14 cm de longitud. Est&aacute; formada de 14 a 22 anillos en forma de herradura; cada uno mide, en promedio, 4 mm de longitud y 1 mm de grosor.<sup>1</sup> Inicia en el borde inferior del cart&iacute;lago cricoides desempe&ntilde;&aacute;ndose como plano de sustentaci&oacute;n de la caja lar&iacute;ngea.<sup>2</sup> El primer anillo traqueal se une al cricoides a trav&eacute;s de la membrana cricotiroidea, este nexo brinda el soporte a la primera porci&oacute;n de la tr&aacute;quea. A partir de esta regi&oacute;n, la tr&aacute;quea se extiende hacia abajo y atr&aacute;s volvi&eacute;ndose intrator&aacute;cica a nivel del manubrio esternal y se bifurca a nivel de la cuarta o quinta v&eacute;rtebra dorsal en los dos bronquios principales.<sup>1,2</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>EPIDEMIOLOG&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2">El punto central del manejo de pacientes con traqueostom&iacute;a se fundamenta en la frecuencia en que se realiza el procedimiento. La posibilidad de someter a un paciente a traqueostom&iacute;a se relaciona directamente con el tiempo bajo ventilaci&oacute;n mec&aacute;nica (VM). Informes previos mencionan que 1 de cada 10 pacientes con apoyo mec&aacute;nico de la respiraci&oacute;n por m&aacute;s de tres d&iacute;as en la Unidad de Cuidados Intensivos (UCI) ser&aacute;n intervenidos a dicho procedimiento.<sup>3</sup> En Estados Unidos anualmente son reportadas m&aacute;s de 100,000 traqueostom&iacute;as, la mayor&iacute;a de ellas en UCI.<sup>4</sup> De &eacute;stas, la serie m&aacute;s grande establece una frecuencia de 5.6% en pacientes con VM.<sup>5</sup> Por otro lado, pa&iacute;ses europeos han informado que la frecuencia oscila entre 1.3 y 10% de los pacientes hospitalizados en UCI.<sup>6</sup> Con respecto a M&eacute;xico, no se cuenta con estad&iacute;sticas confiables sobre la frecuencia de traqueostom&iacute;as, sin embargo, algunos centros reportan entre 13 y 22 procedimientos anuales.<sup>7,8</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">  <b>TIPOS DE C&Aacute;NULAS DE TRAQUEOSTOM&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2">Las c&aacute;nulas de traqueostom&iacute;a pueden ser: a) met&aacute;licas, o b) cloruro de polivinilo (PVC). Las met&aacute;licas carecen del adaptador para integrarse al ventilador, aspecto que limita el uso en escenarios de urgencia;<sup>9</sup> y las de PVC son termol&aacute;biles y se acoplan al ventilador mediante el puerto de adaptaci&oacute;n universal de 15 mm de di&aacute;metro.<sup>10</sup></font></p>     <p align="justify"><font face="verdana" size="2">En general las c&aacute;nulas pueden ser anguladas o curvas con el prop&oacute;sito de mejorar la adaptaci&oacute;n a la tr&aacute;quea. La longitud habitual es de 56 a 90 mm dependiendo de las necesidades del enfermo, aunque existen c&aacute;nulas con longitudes que alcanzan 110 a 130 mm (extralargas).<sup>9,11</sup> M&aacute;s a&uacute;n, existen subtipos para escenarios cl&iacute;nicos espec&iacute;ficos: a) c&aacute;nulas extralargas en su rama proximal, apropiadas en pacientes con cuello grande (p. ej. obesos) y b) c&aacute;nulas extralargas en su rama distal, recomendables para enfermos con traqueomalacia.<sup>9,10</sup> Algunas c&aacute;nulas est&aacute;n reforzadas con alambre flexible en espiral y tienen un borde ajustable para la longitud deseada. Todas las c&aacute;nulas extralargas pueden tener o no incorporado un globo.<sup>9</sup> Tambi&eacute;n existen c&aacute;nulas fenestradas cuyo objetivo es permitir la respiraci&oacute;n comunicada a trav&eacute;s de la v&iacute;a a&eacute;rea superior.<sup>9,10</sup> En la <a href="#a6f1" target="_self">figura 1</a> se muestran los elementos de una traqueostom&iacute;a de PVC.</font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p><a name="a6f1"></a></p>    <p>&nbsp;</p>    <p align="center"><img src="../img/revistas/nct/v73n4/a6f1.jpg"></p>    <p>&nbsp;</p>    <p><font size="2" face="Verdana"></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>TIEMPO IDEAL PARA REALIZAR UNA TRAQUEOSTOM&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2">En el presente, existe una asociaci&oacute;n directa entre el tiempo bajo intubaci&oacute;n orotraqueal y da&ntilde;o a la mucosa respiratoria.<sup>12,13</sup> Los cambios ocurren de manera progresiva e incluso pueden desarrollarse &uacute;lceras en la mucosa traqueal en los primeros 15 minutos de haber colocado un tubo traqueal; m&aacute;s a&uacute;n, de persistir el est&iacute;mulo lesivo (a partir de las 3 horas), puede comprometerse la estructura del cart&iacute;lago traqueal con el desarrollo subsecuente de estenosis y, en casos graves, perforaciones.<sup>14,15</sup> Por fortuna las lesiones agudas no predicen la formaci&oacute;n de cicatriz dado que existe evidencia de un elevado porcentaje de curaci&oacute;n espont&aacute;nea.<sup>16,17</sup> El riesgo de estenosis es proporcional al tiempo bajo intubaci&oacute;n orotraqueal; com&uacute;nmente la frecuencia reportada es del 6% durante la primera semana, pero se duplica luego del d&eacute;cimo d&iacute;a (12%).<sup>18</sup> Con base en estos datos persiste la controversia sobre el momento ideal para realizar una traqueostom&iacute;a, debido a que los desenlaces no han sido contundentes al comparar estrategias tempranas <i>vs.</i> tard&iacute;as.<sup>19</sup></font></p>     <p align="justify"><font face="verdana" size="2">Tomando como referencia el tiempo para realizar una traqueostom&iacute;a, dos metaan&aacute;lisis compararon la evoluci&oacute;n y el desenlace de m&aacute;s de 3,000 pacientes sometidos al procedimiento (menor a 10 d&iacute;as <i>vs.</i> mayor a 10 d&iacute;as); no se encontraron diferencias en tiempo de estancia en UCI, neumon&iacute;a asociada a ventilaci&oacute;n o d&iacute;as bajo VM.<sup>19,20</sup> Continuando la b&uacute;squeda de resultados que confirmen el beneficio de la traqueostom&iacute;a temprana, Liu X <i>et al</i>. reci&eacute;n analizaron los resultados de 11 ensayos cl&iacute;nicos enfoc&aacute;ndolo en dos desenlaces: a) mortalidad a corto plazo y b) neumon&iacute;a asociada a ventilaci&oacute;n mec&aacute;nica (NAVM).<sup>21</sup> Los resultados confirman un beneficio en mortalidad a corto plazo con la estrategia de traqueostom&iacute;a temprana (OR 0.74, IC 95% 0.58-0.95). Con base en las conclusiones de los autores, los resultados a&uacute;n deben aplicarse con cautela pues existen varias limitantes, entre ellas: a) el beneficio se observ&oacute; con el metaan&aacute;lisis acumulado de los &uacute;ltimos tres ensayos cl&iacute;nicos;<sup>22-24</sup> m&aacute;s a&uacute;n, de forma contrastante, el beneficio en mortalidad no se precisa con el an&aacute;lisis individual de cada estudio; b) falta de acuerdo en la definici&oacute;n operativa de traqueostom&iacute;a temprana, c) n&uacute;mero de estudios y poblaci&oacute;n para obtener resultados precisos, d) heterogeneidad entre los criterios de los ensayos cl&iacute;nicos y e) poblaciones heterog&eacute;neas. Es importante resaltar que los estudios enfocados en demostrar el beneficio de la estrategia temprana han mejorado sus criterios de selecci&oacute;n, posiblemente s&oacute;lo es cuesti&oacute;n de tiempo para demostrar con precisi&oacute;n y exactitud el beneficio de dicha intervenci&oacute;n quir&uacute;rgica. Por otro lado, se mantiene la falta de beneficio en disminuir la complicaci&oacute;n de NAVM.<sup>21,25</sup> Los autores del presente manuscrito consideramos que la decisi&oacute;n debe tomarse con base en el an&aacute;lisis individual de cada paciente.</font></p>     <p align="justify"><font face="verdana" size="2">A pesar de los resultados es frecuente que en la pr&aacute;ctica cotidiana se decida someter al paciente a una traqueostom&iacute;a con base en el tiempo estimado que permanecer&aacute; bajo VM.<sup>26,27</sup> Es necesario enfatizar que esta conducta no se encuentra sustentada en ensayos cl&iacute;nicos; la evidencia tangible se remonta a 1989 donde se emiti&oacute; mediante un consenso de expertos, la recomendaci&oacute;n de esperar 21 d&iacute;as antes de tomar la decisi&oacute;n para someter a un paciente a traqueostom&iacute;a.<sup>28</sup> Asimismo, reportes que derivan de ensayos cl&iacute;nicos consideran adecuado no esperar m&aacute;s de 10 d&iacute;as de intubaci&oacute;n orotraqueal a pesar de no existir diferencias s&oacute;lidas en los desenlaces registrados a corto plazo.<sup>22,29</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>COMPLICACIONES DE LA TRAQUEOSTOM&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2">Existen dos modalidades de traqueostom&iacute;a, la quir&uacute;rgica y la percut&aacute;nea.<sup>30</sup> Las complicaciones que derivan del procedimiento se dividen en: <b>inmediatas</b>, <b>mediatas</b> y <b>tard&iacute;as</b>.<sup>30</sup> En el primer grupo (menos de 24 horas) se incluyen: fracaso en el procedimiento, embolismo a&eacute;reo, aspiraci&oacute;n, hemorragia, neumot&oacute;rax, lesi&oacute;n del cart&iacute;lago cricoides, da&ntilde;o quir&uacute;rgico del es&oacute;fago, nervio lar&iacute;ngeo o la c&uacute;pula pleural.<sup>31,32</sup> Entre las mediatas (d&iacute;a 1-7) se encuentran: neumot&oacute;rax, neumomediastino, hemorragia, infecci&oacute;n del estoma, ulceraci&oacute;n del estoma, disfagia, decanulaci&oacute;n accidental, obstrucci&oacute;n de la c&aacute;nula con secreciones, enfisema subcut&aacute;neo, aspiraci&oacute;n y abscesos pulmonares, traque&iacute;tis, traqueobronquitis, atelectasia, desplazamiento de la c&aacute;nula.<sup>31,32</sup> Finalmente, las tard&iacute;as (mayor a 7 d&iacute;as) son: neumon&iacute;a, aspiraci&oacute;n, disfagia, decanulaci&oacute;n accidental, granulomas traqueales, f&iacute;stulas traqueocut&aacute;neas o traqueoesof&aacute;gicas, traqueomalacia y estenosis laringotraqueal.<sup>30-34</sup> La complicaci&oacute;n m&aacute;s frecuente es el sangrado posprocedimiento (5%).<sup>35</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>TRAQUEOSTOM&Iacute;A Y FUNCI&Oacute;N PULMONAR</b></font></p>     <p align="justify"><font face="verdana" size="2">Todo paciente que es sometido a una traqueostom&iacute;a pierde la capacidad para humidificar y calentar el aire inspirado.<sup>36,37</sup> Adem&aacute;s, tiene mayor riesgo de alteraciones en la mec&aacute;nica de degluci&oacute;n y del reflejo tus&iacute;geno (vinculado con mal manejo de secreciones); en conjunto, predisponen a infecciones del tracto respiratorio inferior.<sup>38</sup> Adem&aacute;s, genera disminuci&oacute;n del espacio muerto (entre 60-70 mL) y p&eacute;rdida de la presi&oacute;n positiva al final de la espiraci&oacute;n (mediada por la actividad de la glotis) incidiendo en menor capacidad funcional residual e incrementando el riesgo de atelectasias.<sup>38</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>MANEJO DE LA TRAQUEOSTOM&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2"><b>Cuidados generales</b></font></p>     <p align="justify"><font face="verdana" size="2">La traqueostom&iacute;a debe estar asegurada y no retirarse antes de cinco a siete d&iacute;as. &Uacute;nicamente se podr&aacute; sustituir si existe da&ntilde;o del manguito o cuando el calibre de otra c&aacute;nula ajuste mejor al paciente.<sup>35</sup> La herida debe asearse diariamente con soluci&oacute;n salina al 0.9% bajo t&eacute;cnica de asepsia y antisepsia (guantes, campos est&eacute;riles y gasas simples); es imperativo evitar en todo momento laceraciones en la piel. La endoc&aacute;nula debe limpiarse tantas veces sea necesario para evitar ac&uacute;mulo de secreciones en su interior y obstrucci&oacute;n subsecuente; la t&eacute;cnica de lavado incluye soluci&oacute;n salina al 0.9% en conjunto con cepillos dise&ntilde;ados para la endoc&aacute;nula; despu&eacute;s debe sumergirse durante 20 minutos en glutaraldeh&iacute;do al 8.2% y retirar el exceso del desinfectante con soluci&oacute;n salina antes de recolocarla.<sup>11</sup> Asimismo, es recomendable el uso temprano de las v&aacute;lvulas de fonaci&oacute;n debido a que mejoran la comunicaci&oacute;n del enfermo y contribuyen a recuperar la funci&oacute;n de las estructuras faringolar&iacute;ngeas por incremento de la presi&oacute;n subgl&oacute;tica, al favorecer el uso de la musculatura intr&iacute;nseca de la laringe.<sup>39</sup></font></p>     <p align="justify"><font face="verdana" size="2">El globo de la traqueostom&iacute;a tiene dos funciones principales: prevenir la aspiraci&oacute;n de secreciones y optimizar su drenaje.<sup>9</sup> Debe ser monitorizado continuamente para mantener una presi&oacute;n que oscile entre 20 y 25 mmHg; valores superiores sobrepasan la presi&oacute;n de perfusi&oacute;n capilar en la mucosa traqueal con el consecuente riesgo de isquemia y estenosis.<sup>14,15</sup> Cuando la presi&oacute;n es menor de 20 mmHg, el globo forma pliegues longitudinales que favorecen la formaci&oacute;n de <i>biofilm</i> con un riesgo cuatro veces superior para desarrollar neumon&iacute;a asociada al ventilador.<sup>40,41</sup></font></p>     <p align="justify"><font face="verdana" size="2">Existen tres tipos de manguitos: a) de alto volumen y baja presi&oacute;n (se inflan con aire y son los m&aacute;s usados); b) de bajo volumen y alta presi&oacute;n (indicados en casos que requieran desinflado intermitente y emplean soluci&oacute;n salina para inflarlos); c) de hule-espuma (cuyo material propicia una tendencia a mantenerlo inflado; m&aacute;s a&uacute;n, el grado de expansi&oacute;n es influenciado por la presi&oacute;n traqueal).<sup>9</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Humidificaci&oacute;n</b></font></p>     <p align="justify"><font face="verdana" size="2">En condiciones normales el aire inspirado se humidifica y calienta conforme avanza en la v&iacute;a a&eacute;rea y, una vez que alcanza la temperatura corporal de 37 <sup>o</sup>C, su humedad relativa es del 100%.<sup>42</sup> A este punto se le denomina l&iacute;mite de saturaci&oacute;n isot&eacute;rmica (ISB, por sus siglas en ingl&eacute;s) y su referencia anat&oacute;mica se localiza a 5 cm debajo de la carina principal.<sup>43</sup> La traqueostom&iacute;a modifica el ISB en el &aacute;rbol bronquial desplaz&aacute;ndolo a una regi&oacute;n no capacitada (bronqu&iacute;olos respiratorios) para desempe&ntilde;ar dichas funciones.<sup>42,44</sup></font></p>     <p align="justify"><font face="verdana" size="2"><b>Repercusiones de humidificaci&oacute;n inadecuada</b></font></p>     <p align="justify"><font face="verdana" size="2">La respiraci&oacute;n de aire seco contribuye con p&eacute;rdida de vapor de agua a trav&eacute;s de la mucosa bronquial;<sup>42</sup> como consecuencia, deshidrataci&oacute;n del epitelio, aumento en la densidad de la capa de moco e incapacidad del aparato mucociliar para arrastrar las secreciones.<sup>45</sup> El efecto del exceso de humedad es similar.<sup>46</sup></font></p>     <p align="justify"><font face="verdana" size="2">A nivel histol&oacute;gico ocurre metaplasia escamosa, da&ntilde;o a las gl&aacute;ndulas mucosas, aplanamiento y desorganizaci&oacute;n de las c&eacute;lulas ciliadas (<a href="#a6f2" target="_self">figura 2</a>).<sup>47</sup> Estos cambios favorecen retenci&oacute;n de secreciones y aparici&oacute;n de atelectasias; funcionalmente se caracteriza por disminuci&oacute;n de la capacidad vital forzada (FVC) y distensibilidad est&aacute;tica, incremento variable en la resistencia de la v&iacute;a a&eacute;rea y aparici&oacute;n de hipoxemia con gradiente alv&eacute;olo-arterial elevado.<sup>42,47-50</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p><a name="a6f2"></a></p>    <p>&nbsp;</p>    <p align="center"><img src="../img/revistas/nct/v73n4/a6f2.jpg"></p>    <p>&nbsp;</p>    ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana"></font></p>     <p align="justify"><font face="verdana" size="2">Suzukawa <i>et al.</i>, propusieron una escala simple para definir las caracter&iacute;sticas f&iacute;sicas de las secreciones. Con base en los resultados se pueden tomar decisiones para ajustar la humedad en un paciente con v&iacute;a a&eacute;rea artificial (<a href="#a6t1" target="_self">tabla 1</a>).<sup>51</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p><a name="a6t1"></a></p>    <p>&nbsp;</p>    <p align="center"><img src="../img/revistas/nct/v73n4/a6t1.jpg"></p>    <p>&nbsp;</p>    <p><font size="2" face="Verdana"></font></p>     <p align="justify"><font face="verdana" size="2"><b>Tipos de humidificadores</b></font></p>     <p align="justify"><font face="verdana" size="2">Un humidificador es un dispositivo que a&ntilde;ade mol&eacute;culas de agua a un gas. Se clasifican en pasivos o activos. Los activos utilizan una fuente externa de calor y agua; en tanto los pasivos utilizan la temperatura y humedad que provienen de la respiraci&oacute;n del paciente.<sup>42</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Activos</b></font></p>     <p align="justify"><font face="verdana" size="2"><i>Humidificadores de agua caliente.</i> Existen diferentes tipos: generadores de burbujas, <i>passover</i>, contraflujo e inyecci&oacute;n de vapor. Todos brindan humedad absoluta disponible entre 35 y 40 mgH<sub>2</sub>O/L. Desventajas: quemadura potencial de la v&iacute;a a&eacute;rea y condensaci&oacute;n de los sistemas de conducci&oacute;n.<sup>11</sup></font></p>     <p align="justify"><font face="verdana" size="2"><i>Humidificadores de agua fr&iacute;a:</i> el modelo tradicional es el Puritan&trade;.<sup>52</sup> Este sistema genera entre 34 y 44 mgH<sub>2</sub>O/L de humedad absoluta a temperaturas fr&iacute;as; posee m&uacute;ltiples desventajas: incremento parad&oacute;jico en volumen de secreciones, f&aacute;cil contaminaci&oacute;n y potencial transmisi&oacute;n de agentes infecciosos. Los autores consideramos que no deben emplearse, exceptuando situaciones en donde no se pueda proporcionar otro sistema de humidificaci&oacute;n.<sup>53</sup></font></p>     <p align="justify"><font face="verdana" size="2"><b>Pasivos</b></font></p>     <p align="justify"><font face="verdana" size="2"><i>Intercambiadores de calor y humedad</i> (<i>humidity moisture exchangers</i> o HME por sus siglas en ingl&eacute;s), conocidos como "nariz artificial".<sup>54</sup> Estos dispositivos facilitan la retenci&oacute;n de calor y humedad disponibles para el siguiente ciclo inspiratorio;<sup>55</sup> pueden generar una humedad absoluta entre 30 y 32 mgH<sub>2</sub>O/L a una temperatura que oscila entre 27 y 30 <sup>o</sup>C.<sup>47</sup> Se recomienda cambiarlos cada 24 a 48 horas, y en casos muy seleccionados pueden mantenerse hasta una semana.<sup>56</sup> Desventajas: Incrementan el espacio muerto, aumentan la resistencia en el ciclo de inspiraci&oacute;n/espiraci&oacute;n y aumentan el PEEP intr&iacute;nseco.<sup>57</sup> Las contraindicaciones incluyen: casos con secreciones espesas, f&iacute;stulas broncopleurales, fuga en el globo de la traqueostom&iacute;a, entre otros.<sup>11,42</sup></font></p>     <p align="justify"><font face="verdana" size="2"><i>Cable calefactor: </i>brinda calor mediante electricidad al circuito del ventilador, proporciona una temperatura entre 37 y 40 <sup>o</sup>C.<sup>58</sup> Su eficiencia disminuye con el incremento en la temperatura ambiental y del dep&oacute;sito. En diversos estudios se ha demostrado que no hay diferencia en la incidencia de neumon&iacute;a y riesgo de oclusiones cuando se compara con un HME.<sup>59</sup></font></p>     <p align="justify"><font face="verdana" size="2"><i>Elecci&oacute;n del humidificador</i>. No existe un m&eacute;todo universal de humidificaci&oacute;n; sin embargo, debe proporcion&aacute;rsele a todos los enfermos con v&iacute;a a&eacute;rea artificial.<sup>42</sup> La humedad absoluta que genera cada dispositivo se basa en pruebas <i>in vitro</i>, aspecto que limita conocer con exactitud el nivel de humedad que otorga el m&aacute;ximo beneficio en la pr&aacute;ctica cl&iacute;nica. Se considera que la elecci&oacute;n del sistema de humidificaci&oacute;n debe ser con base en: 1) escenario cl&iacute;nico, 2) recursos disponibles y 3) capacidad del dispositivo para proporcionar un nivel apropiado de humedad salvaguardando la integridad de cada paciente.<sup>60-62</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>DRENAJE DE SECRECIONES</b></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">La aspiraci&oacute;n de secreciones a trav&eacute;s de la c&aacute;nula de traqueostom&iacute;a es inc&oacute;moda para el paciente y potencialmente peligrosa;<sup>35</sup> com&uacute;nmente es realizada con base en la percepci&oacute;n del servicio param&eacute;dico (enfermer&iacute;a, t&eacute;cnicos en terapia respiratoria). La Academia Americana de Cuidados Respiratorios (AARC, por sus siglas al ingl&eacute;s) ha desarrollado gu&iacute;as de pr&aacute;ctica cl&iacute;nica para orientar la toma de decisiones; estos lineamientos sugieren que se lleve a cabo una vez por turno, particularmente en situaciones como: a) roncus en la auscultaci&oacute;n de tr&aacute;quea y t&oacute;rax; b) deterioro en el nivel de oxigenaci&oacute;n (oximetr&iacute;a de pulso o gasometr&iacute;a); c) elevaci&oacute;n de la presi&oacute;n pico durante VM; d) incremento del trabajo respiratorio; e) secreciones visibles en el tubo traqueal; otros escenarios clasificados como "de rutina" son muy cuestionables.<sup>63</sup> Reci&eacute;n, un reporte incorpor&oacute; un sensor ac&uacute;stico al circuito del ventilador, el sensor emite una alerta para justificar la aspiraci&oacute;n de secreciones. La adici&oacute;n del dispositivo disminuye la frecuencia de succiones innecesarias en comparaci&oacute;n con el grupo control (4% <i>vs.</i> 12%; p &lt; 0.001).<sup>64</sup> Adem&aacute;s, se debe escoger el di&aacute;metro adecuado de las sondas de aspiraci&oacute;n para cada tama&ntilde;o de traqueostom&iacute;a.<sup>11,64-66</sup> La <a href="../img/revistas/nct/v73n4/a6t2.jpg" target="_blank">tabla 2</a> muestra las indicaciones cl&iacute;nicas para llevar a cabo el drenaje de secreciones.<sup>11</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>CAMBIO DE LA C&Aacute;NULA DE TRAQUEOSTOM&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2">El primer cambio de la traqueostom&iacute;a es cada vez que el estoma ha madurado siendo aconsejable luego de 7 a 14 d&iacute;as; con esta modificaci&oacute;n, se logra mejor tolerancia a la v&iacute;a oral, fonaci&oacute;n y menor estancia intrahospitalaria.<sup>67,68</sup> Es prudente que el cambio se realice entre dos personas expertas en el manejo de traqueostom&iacute;as, debido a que existe un riesgo de erosionar la arteria innominada o crear una falsa v&iacute;a, particularmente cuando su posici&oacute;n es no central.<sup>3,68</sup></font></p>     <p align="justify"><font face="verdana" size="2">Con base en la b&uacute;squeda de evidencia actual, no existe un consenso que establezca la frecuencia de los cambios de c&aacute;nula de traqueostom&iacute;a en pacientes con uso prolongado.<sup>36</sup> Algunos autores recomiendan cada tres meses<sup>69</sup> y los fabricantes mencionan intervalos muy variables (1 a 3 meses). Derivado de la informaci&oacute;n disponible, consideramos que los cambios a largo plazo pudieran realizarse cada 8 a 12 semanas.<sup>70</sup> En adici&oacute;n, los centros con mayor experiencia sugieren que se lleven a cabo bajo visi&oacute;n endosc&oacute;pica para detectar posibles complicaciones inherentes a la c&aacute;nula, corroborar su posici&oacute;n y valorar la totalidad de la v&iacute;a a&eacute;rea.<sup>36,71</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>RETIRO DE LA TRAQUEOSTOM&Iacute;A</b></font></p>     <p align="justify"><font face="verdana" size="2">Una de las recomendaciones para retirar la traqueostom&iacute;a consiste en disminuir progresivamente el di&aacute;metro de la c&aacute;nula. Dicha estrategia, sumada al uso de v&aacute;lvulas para fonaci&oacute;n, permiten recuperar las estructuras lar&iacute;ngeas.<sup>72</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">No conocemos la frecuencia de decanulaci&oacute;n no planeada. Todo procedimiento que se realice en situaciones cl&iacute;nicas con decanulaci&oacute;n fortuita debe considerar el tiempo de evoluci&oacute;n del estoma. En situaciones que ameriten recolocaci&oacute;n de urgencia, sobre todo estomas con menos de siete d&iacute;as de evoluci&oacute;n se recomienda lo siguiente: 1) verificar adecuada saturaci&oacute;n por oximetr&iacute;a y/o proporcionar ox&iacute;geno suplementario; 2) el intento de recolocaci&oacute;n es con una c&aacute;nula del mismo tama&ntilde;o o m&aacute;s peque&ntilde;a, apropiadamente lubricada, valorando la posici&oacute;n del estoma; de no ser factible, deber&aacute; guiarse por broncoscop&iacute;a.<sup>39</sup> En caso de que la traqueostom&iacute;a cuente con suturas/riendas, tirar de ellas gentilmente para evitar mayor trauma y as&iacute; favorecer la inserci&oacute;n de la c&aacute;nula;<sup>3</sup> 3) corroborar posici&oacute;n en tr&aacute;quea y sujetarla; 4) de persistir con inadecuada saturaci&oacute;n de ox&iacute;geno o no se logre recolocar la c&aacute;nula, es aconsejable tener disponible alg&uacute;n dispositivo que otorgue ox&iacute;geno a mayor concentraci&oacute;n y realizar una intubaci&oacute;n translar&iacute;ngea.<sup>67</sup> En el caso de los estomas maduros (m&aacute;s de 7 d&iacute;as) el abordaje es similar.<sup>39</sup> Por otro lado, para reforzar lo previamente descrito en estomas con tiempo menor a siete d&iacute;as y emergencia ventilatoria, remitimos al lector a consultar la literatura especializada en donde se establecen lineamientos ampliamente aceptados.<sup>73</sup></font></p>     <p align="justify"><font face="verdana" size="2">Existen diversos protocolos de decanulaci&oacute;n.<sup>50,72,74-76</sup> La c&aacute;nula de traqueostom&iacute;a es retirada cuando existe permeabilidad de la v&iacute;a a&eacute;rea y se ha resuelto el problema primario.<sup>73</sup> Ceriana <i>et al</i>., hacen &eacute;nfasis en el cumplimiento de las siguientes condiciones para su retiro: 1) saturaci&oacute;n de ox&iacute;geno al aire ambiente mayor a 92%; 2) ausencia de secreciones abundantes; 3) reflejo tus&iacute;geno adecuado con flujo pico de tos mayor o igual a 160 L/min; 4) radiograf&iacute;a de t&oacute;rax sin anormalidades (neumon&iacute;a, neumot&oacute;rax, etc.); 5) evaluar de manera individual la decisi&oacute;n de someter a broncoscop&iacute;a para descartar estenosis laringo-traqueal.<sup>39,74-77</sup></font></p>     <p align="justify"><font face="verdana" size="2">Con base en la literatura disponible no conocemos un consenso sobre el volumen de esputo que defina la presencia de secreciones copiosas en pacientes con v&iacute;a a&eacute;rea artificial.<sup>78</sup> Desde el punto de vista fisiol&oacute;gico, un adulto sano produce entre 10 y 100 mL de moco por d&iacute;a en la v&iacute;a a&eacute;rea; aunque estos datos son muy variables en pacientes con traqueostom&iacute;a.<sup>78</sup> Se acepta que una cantidad mayor de 120 mL/d&iacute;a o m&aacute;s de 5 mL/h (p. ej., traqueobronquitis) es anormal y amerita ajuste en la humedad y frecuencia de aspiraci&oacute;n.<sup>78,79</sup> El impacto de esta variable en la decisi&oacute;n cl&iacute;nica de decanular subyace en un buen reflejo tus&iacute;geno y la viscosidad de las secreciones.<sup>39</sup></font></p>     <p align="justify"><font face="verdana" size="2">En adici&oacute;n, es aconsejable realizar una evaluaci&oacute;n completa del mecanismo de degluci&oacute;n previo a decanular al paciente.<sup>76</sup> Un m&eacute;todo validado consiste en otorgar una mezcla de agua y azul de metileno en un volumen total de 50 mL. Durante el proceso se deber&aacute; desinflar totalmente el globo. Se confirma aspiraci&oacute;n y, como consecuencia, posibles trastornos en la motilidad esof&aacute;gica (<i>v.gr.</i> disfagia) cuando expectora el colorante a trav&eacute;s de la c&aacute;nula. Toda prueba positiva, justifica detener el protocolo de decanulaci&oacute;n y referir al especialista apropiado para descartar la presencia de f&iacute;stula traqueoesof&aacute;gica.<sup>76</sup></font></p>     <p align="justify"><font face="verdana" size="2">Finalmente, la decisi&oacute;n de retirar una c&aacute;nula de traqueostom&iacute;a con base en el desempe&ntilde;o del paciente posterior a ocluir la c&aacute;nula es muy debatible y con frecuencia se lleva a cabo el proceso en la pr&aacute;ctica cl&iacute;nica cotidiana.<sup>80,81</sup> Hasta hoy no existe un ensayo cl&iacute;nico adecuadamente dise&ntilde;ado para respaldar este procedimiento. Existe informaci&oacute;n que deriva de encuestas a m&eacute;dicos donde se reporta que en general, consideran importante la tolerancia a la oclusi&oacute;n de la c&aacute;nula para decidir su retiro, siempre y cuando el manguito est&eacute; desinflado en el caso de c&aacute;nulas no fenestradas.<sup>39,82</sup> Derivado de un consenso cl&iacute;nico qued&oacute; establecido que a todo paciente, previo al egreso hospitalario, deben retir&aacute;rsele las suturas de la traqueostom&iacute;a.<sup>39</sup></font></p>     <p align="justify"><font face="verdana" size="2"></font></p>    <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>    <p align="justify"><font face="verdana" size="2">  <b>CONCLUSIONES</b></font></p>     <p align="justify"><font face="verdana" size="2">Acorde con la evidencia actual, el tiempo &oacute;ptimo para realizar una traqueostom&iacute;a en pacientes bajo VM est&aacute; encaminado hacia una estrategia temprana; posiblemente, los datos se confirmen en estudios subsecuentes y el an&aacute;lisis sistem&aacute;tico de los mismos. En tanto, consideramos que la decisi&oacute;n debe basarse en evaluaciones individuales y con estricto apego a la evidencia s&oacute;lida.</font></p>     <p align="justify"><font face="verdana" size="2">Los cuidados generales de la traqueostom&iacute;a impactan en el desenlace de los pacientes. Es mandatorio iniciar o continuar los lineamientos de pr&aacute;ctica cl&iacute;nica establecidos a nivel internacional y desempe&ntilde;arlos estrictamente en cada instituci&oacute;n de asistencia m&eacute;dica; tales como: rutina de higiene de la c&aacute;nula, humedad, manejo de secreciones, registro de la presi&oacute;n del globo, asistencia integral durante eventos no planeados de decanulaci&oacute;n y retiro planeado de la traqueostom&iacute;a.</font></p>     ]]></body>
