Sin embargo, de manera reciente se ha involucrado la valoración de la cooximetría, en especial la carboxihemoglobina, al lado de la cama del paciente críticamente enfermo como variable relevante en la inflamación y la apoptosis. La hiperrespuesta que acompaña la leptospirosis seguida de la fase de inmunoparálisis liderada por la cinasa 4 de la interleuquina 1 (IRAK 4) ocasiona dos fenómenos fisiológicos sumamente interesantes: primero, contrarregula el mitógeno p37 de la proteína cinasa (MAPKp37), y segundo, modula la marginación, diapédesis, adherencia y fagocitosis del monocito, lo que genera una inducción masiva de hemo-oxigenasa-1 (HO-1) aportando incremento en la carboxihemoglobina (COHb) como vía alterna en su producción al margen de la liberada en el pulmón. Presentamos el caso clínico de un paciente infectado por espiroquetas cuyas concentraciones de carboxihemoglobina fueron elevadas sin determinantes pulmonares relacionados.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract: Arterial gas assessment provides critical information to the interior of intensive care units. However, recently the assessment of co-oximetry, especially carboxyhemoglobin, has been involved at the bedside of the critically ill patient as a variable relevant in inflammation and apoptosis. The accompanying hyperresponse leptospirosis followed by the immunoparalysis phase led by kinase 4 of interleukin 1 (IRAK 4) causes two highly important physiological phenomena Interesting: first, it counterregulates the protein kinase P37 mitogen (MAPKp37), and second, it modulates marginalization, diapedesis, adherence and phagocytosis of the monocyte, which generates a massive induction of heme oxygenase-1 (HO-1) providing an increase in carboxyhemoglobin (COHb) as an alternative route in its production apart from that released in the lung. Introducing the clinical case of a patient infected by spirochetes whose concentrations carboxyhemoglobin were elevated with no related pulmonary determinants.]]></p></abstract>
<abstract abstract-type="short" xml:lang="pt"><p><![CDATA[Resumo: A avaliação dos gases sanguíneos arteriais fornece informações importantes nas unidades de terapia intensiva; no entanto, recentemente, a avaliação da co-oximetria, especialmente da carboxiemoglobina, à beira do leito do paciente em estado crítico, foi apontada como uma variável relevante na inflamação e na apoptose. A hiper-resposta que acompanha a leptospirose, seguida pela fase de imunoparalisia liderada pela quinase 4 da interleucina-1(IRAK 4), causa dois fenômenos fisiológicos altamente interessantes: primeiro, ela contra-regula o mitógeno p37 da proteína quinase (MAPKp37), e, em segundo lugar, modula a marginação, a diapedese, a adesão e a fagocitose dos monócitos, gerando uma indução maciça de heme oxigênio 1 (HO-1) e contribuindo para o aumento da carboxiemoblobina (COHb) como uma via alternativa para sua produção, além da liberada no pulmão.
