El objetivo del presente estudio fue evaluar su repercusión en el neurodesarrollo a los tres años de edad. Material y métodos: diagnóstico de hemorragia intraventricular en cualquiera de sus grados durante el periodo neonatal, según clasificación de Papile por medio de ultrasonido transfontanelar, con valoración neurológica a los tres años de edad. Se estudiaron 124 niños, ordenados en cuatro grupos conforme al grado de hemorragia: grado I, 14 pacientes; grado II, 84; grado III 22, y grado IV, 9. Resultados: la morbilidad fue semejante en todos los grupos, sólo la hidrocefalia predominó con 73% para el grado III y la enfermedad crónica pulmonar en el grado IV con 77%. la muestra presentó 45% de alteraciones neurológicas, el mayor porcentaje fue para el grado IV con 89%, con razón de momios para los grados de hemorragia I1I/IV de 3.51IC 95% 1.49-8.28 p = 0.005. la escala de Terman Merril (I.Q) mostró puntajes por abajo del promedio para los grados II y III y para la grado IV el 45% presentó retraso mental. Para la audición no existieron diferencias estadísticamente significativas. Conclusión: a mayor grado de hemorragia las alteraciones neurológicas fueron más frecuentes, con riesgo aumentado para los grados III/IV. Para la hemorragia grado IV el 45% de los pacientes tuvieron retraso mental. las patologías con frecuencia significativa para la hemorragia grado III/ IV fueron la hidrocefalia y la enfermedad pulmonar crónica.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Intraventricular and subependymal hemorrhage are common complications among preterm infants. The aim of the present study was to assess its implication in the neurodevelopment at 3 years of age. Material and methods: cerebral ultrasonography with evidence of any stage of intraventricular hemorrhage (Papule's classification) during neonatal period and a neurologic evaluation at 3 years of age were performed on each patient. One hundred twenty four patients were enrolled and assigned to four groups according the intraventricular hemorrhage (IVH) stage: Grade I, 14 patients, grade II, 84, grade III, 22, and grade Wnine patients. Results: morbility was similar for all groups; hydrocephalus was more frequent in group III (73%), and in group IV chronic pulmonary disease (77%) was more frequent. Forty five percent of the sample showed neurologic alterations with the greatest proportion in group IV (89%); OR for groups III and IV was 3.51 with a 95% confidence interval of (l. 49, 8.28p= 0.005). Grades III&IV showed lowest Terman Merril scores and grade W scores were within the range of mental retardation. No statistical difference was found between grades at audition evaluation. Conclusions: neurologic alterations were greater in poorer grades of IVH, and risk will increment for grades III and IV. La utilizaci&oacute;n de ultrasonido transfontanelar permite su detecci&oacute;n temprana y delimitaci&oacute;n anat&oacute;mica espec&iacute;fica<sup>1,2</sup> Su incidencia en la d&eacute;cada pasada se informa de 40 a 60%; los estudios recientes informan 40% en reci&eacute;n nacidos prematuros por debajo de 1500g de peso al nacer y los &uacute;ltimos refieren 20%<sup>3&#150;7</sup></font></p>     <p align="justify"><font face="verdana" size="2">El grado de hemorragia se relaciona en el corto plazo con mortalidad y entre las secuelas la mas frecuente en el grado III y IV es la hidrocefalia. La mortalidad para la hemorragia grado III es de 10% y presentan hidrocefalia 20%; para la grado IV la mortalidad es de 50 a 60% y presentan hidrocefalia de 65 a 100%.<sup>8,9</sup></font></p>     <p align="justify"><font face="verdana" size="2">A largo plazo el desarrollo neurol&oacute;gico en pacientes con hemorragias grado I y II, se relaciona con trastornos transitorios o alteraciones sutiles durante el primer a&ntilde;o de edad posnatal, los cuales con frecuencia se normalizan al final del mismo. Se observan principalmente alteraciones en el tono, (hipertonia o hipotonia) la coordinaci&oacute;n motora (tanto fina como gruesa) y los reflejos.<sup>1011</sup> En la etapa preescolar hasta el 87% son normales, y el 6.5% contin&uacute;a con secuelas menores en tono y/o reflejos.<sup>12</sup> Con relaci&oacute;n al cociente intelectual (IQ) se informan alteraciones leves con puntajes de 71 a 84. Tambi&eacute;n se ha informado el mismo porcentaje (6.5) para secuelas mayores que incluyen cualquier forma de par&aacute;lisis cerebral, IQ menor de 70, hipoacusia con necesidad de auxiliar auditivo, ceguera uni o bilateral secundaria a fibroplasia retrolental.<sup>1314</sup></font></p>     <p align="justify"><font face="verdana" size="2">Durante los a&ntilde;os escolares en el &aacute;rea cognoscitiva, no se encuentran diferencias significativas entre los ni&ntilde;os con ultrasonograf&iacute;a transfontanelar normal y hemorragias grado l/ll, aunque &eacute;stos tienen puntajes m&aacute;s bajos en todas las valoraciones.<sup>15</sup></font></p>     <p align="justify"><font face="verdana" size="2">En los ni&ntilde;os con hemorragia grados III o IV las valoraciones neurol&oacute;gicas para el primer a&ntilde;o de vida no son normales, y mejoran considerablemente al final del mismo; sin embargo los que tienen hidrocefalia o leucomalacia persisten con alteraciones. Aproximadamente 40%, referido como normal puede tener alteraciones sutiles en etapas posteriores cuando a la hemorragia se asocia al peso muy bajo para la edad gestacional. Durante la etapa preescolar, entre 14 a 39% tienen desarrollo normal, algunos secuelas menores de 11 a 14% y secuelas mayores de 50 a 72%.<sup>6 </sup>La par&aacute;lisis cerebral casi siempre est&aacute; presente en estos grados y en particular en ni&ntilde;os con leucomalacia peri&#150;ventricular.<sup>1718</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">Estudios de las &uacute;ltimas d&eacute;cadas con ni&ntilde;os de edad escolar y antecedente de hemorragia severa, (Grado IV) informan anormalidades neurol&oacute;gicas, disfunciones cognoscitivas, da&ntilde;o neuromotor, d&eacute;ficit en el lenguaje, la atenci&oacute;n, el aprendizaje y en las destrezas manuales, hiperactividad y dificultades motoras, visuales e hipoacusia bilateral sensorio&#150;neural.<sup>19,20</sup></font></p>     <p align="justify"><font face="verdana" size="2">La dilataci&oacute;n ventricular y la atrofia cerebral han demostrado ser predictores independientes significativos de deterioro del neurodesarollo, desempe&ntilde;o escolar o aprovechamiento.<sup>21&#150;23</sup></font></p>     <p align="justify"><font face="verdana" size="2">El objetivo del presente estudio es evaluar la repercusi&oacute;n a los tres a&ntilde;os de edad de la hemorragia subependimaria intraventricular en el neurodesarollo en un grupo de ni&ntilde;os nacidos en el Instituto Nacional de Perinatolog&iacute;a de M&eacute;xico.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>Material y m&eacute;todos</b></font></p>     <p align="justify"><font face="verdana" size="2">El estudio se realiz&oacute; en ni&ntilde;os pertenecientes al departamento de seguimiento pedi&aacute;trico del Instituto Nacional de Perinatolog&iacute;a. El Instituto es un centro de atenci&oacute;n de embarazos de alto riesgo con 6000 nacimientos al a&ntilde;o. Cuenta con 16 cunas de terapia intensiva neonatal donde ingresan anualmente entre 350 a 400 reci&eacute;n nacidos. Los criterios de inclusi&oacute;n fueron:</font></p>     <p align="justify"><font face="verdana" size="2">a) Haber nacido en el Instituto.</font></p>     <p align="justify"><font face="verdana" size="2">b) Contar con ultrasonido transfontanelar corroborando hemorragia subependimaria intraventricular en cualquiera de sus grados, seg&uacute;n la clasificaci&oacute;n de Papile.<sup>24</sup></font></p>     <p align="justify"><font face="verdana" size="2">c) Con valoraci&oacute;n neurol&oacute;gica, audiol&oacute;gica y psicol&oacute;gica a los tres a&ntilde;os de edad,</font></p>     <p align="justify"><font face="verdana" size="2">De exclusi&oacute;n:</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">a) Malformaciones mayores.</font></p>     <p align="justify"><font face="verdana" size="2">b) No aceptaci&oacute;n de los padres.</font></p>     <p align="justify"><font face="verdana" size="2">Las variables de estudio fueron: edad gestacional, peso, Apgar, d&iacute;as de ox&iacute;geno, morbilidad neonatal, condici&oacute;n neurol&oacute;gica, audiol&oacute;gica de lenguaje y psicol&oacute;gica.</font></p>     <p align="justify"><font face="verdana" size="2">Se estudiaron a 129 ni&ntilde;os, ordenados en cuatro grupos conforme al grado de hemorragia; grado I, 14 pacientes, grado II, 84, grado III, 22 y grado IV, 9.</font></p>     <p align="justify"><font face="verdana" size="2">El estudio ultrasonogr&aacute;fico fue realizado en todos los casos por una sola persona experta en ultrasonograf&iacute;a cerebral. El diagn&oacute;stico se realiz&oacute; durante la primer semana de vida.</font></p>     <p align="justify"><font face="verdana" size="2">Los instrumentos de valoraci&oacute;n utilizados fueron:</font></p>     <p align="justify"><font face="verdana" size="2">1. Exploraci&oacute;n neurol&oacute;gica de Mayo.<sup>26</sup> Se clasificaron como normales cuando tuvieron buenos resultados en todos los indicadores de la prueba y como alterados cuando tuvieron resultados anormales en dos o m&aacute;s indicadores.</font></p>     <p align="justify"><font face="verdana" size="2">2. Valoraci&oacute;n audiom&eacute;trica, se realiz&oacute; de los seis a 24 meses mediante audiometr&iacute;a de campo, con b&uacute;squeda de reflejo de orientaci&oacute;n auditivo visual con un audi&oacute;metro MAICO. Despu&eacute;s de los dos a&ntilde;os se realiz&oacute; audiometr&iacute;a tonal utilizando frecuencias de 125 a 8000 Hz en una c&aacute;mara sono amortiguada de dos por dos metros.</font></p>     <p align="justify"><font face="verdana" size="2">Se consider&oacute; normal cuando se localiz&oacute; la fuente sonora en uno y otro lados entre 10 a 40 dB.<sup>26+</sup></font></p>     <p align="justify"><font face="verdana" size="2">3. El lenguaje se evalu&oacute; mediante la gu&iacute;a elaborada del servicio de comunicaci&oacute;n humana del Instituto; considerando como normal al que cumpli&oacute; con los requisitos para la edad y retraso cuando se encontr&oacute; tres meses por debajo de la edad real.<sup>27</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">4. Valoraci&oacute;n Terman Merril. Eval&uacute;a cuatro &aacute;reas: razonamiento verbal, abstracci&oacute;n visual, razonamiento num&eacute;rico y memoria a corto plazo, Se clasifica de acuerdo al puntaje en:</font></p>     <p align="justify"><font face="verdana" size="2">&#150;&nbsp; muy superior con un IQ &gt;132</font></p>     <p align="justify"><font face="verdana" size="2">&#150;&nbsp;superior con IQ 121 &#150; 131</font></p>     <p align="justify"><font face="verdana" size="2">&#150;&nbsp; por arriba del promedio IQ 111&#150; 120</font></p>     <p align="justify"><font face="verdana" size="2">&#150;&nbsp; promedio IQ 89 &#150; 110</font></p>     <p align="justify"><font face="verdana" size="2">&#150;&nbsp; por abajo del promedio IQ 79 &#150; 88</font></p>     <p align="justify"><font face="verdana" size="2">&#150;&nbsp; lento aprendizaje IQ 68&#150;78</font></p>     <p align="justify"><font face="verdana" size="2">&#150;&nbsp; retardo mental IQ &lt; 67</font></p>     <p align="justify"><font face="verdana" size="2">Para el an&aacute;lisis de los datos se utilizaron frecuencia, porcentajes, medias, desviaci&oacute;n est&aacute;ndar Ji cuadrada, ANOVA y raz&oacute;n de momios con un nivel de confianza de 95%.</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2"><b>Resultados</b></font></p>     <p align="justify"><font face="verdana" size="2">El promedio de edad gestacional fue de 31 semanas con una desviaci&oacute;n est&aacute;ndar (d.e) de 2.1 y peso promedio de 1420g d.e. 511 g; la distribuci&oacute;n de peso por subgrupos fue semejante para hemorragia grado I, (1338g, d.e 375g) y IV (1364g, d.e. 352g). El puntaje de Apgar al minuto fue m&aacute;s bajo para los grados III y IV; a los cinco minutos m&aacute;s del 70% de los grados I a III tuvo Apgar mayor de siete, mientras que para el grado IV s&oacute;lo 44%.</font></p>     <p align="justify"><font face="verdana" size="2">La media de d&iacute;as de ox&iacute;geno fue de 46 d.e. 36.7, La mayor fue para la grado III, 50 d&iacute;as d.e. 31.9 y grado IV, con 60 d&iacute;as d.e. 37.5.</font></p>     <p align="justify"><font face="verdana" size="2">La morbilidad fue semejante en todos los grupos, s&oacute;lo la hidrocefalia tuvo una frecuencia mayor 73% en la grado III, y la enfermedad cr&oacute;nica pulmonar en la grado IV, con 77%. En la muestra, 45 % present&oacute; alteraciones neurol&oacute;gicas (de leves a graves). En el <a href="../img/revistas/gmm/v140n4/a1c1.jpg" target="_blank">cuadro I</a> se observa, c&oacute;mo a mayor grado de hemorragia, mayor n&uacute;mero de alteraciones. El mayor porcentaje de alteraciones graves se encontr&oacute; en la grado IV con 89%, (p &lt; 0.05) predominando la cuadriparesia y la espasticidad, raz&oacute;n de momios (RM) para hemorragia III/IV 3.51 IC 95% 1.49&#150;8.28 p = 0.005 (<a href="../img/revistas/gmm/v140n4/a1c4.jpg" target="_blank">cuadro IV</a>).</font></p>     <p align="justify"><font face="verdana" size="2">El puntaje global de la escala de Terman Merril para la hemorragia grado I se encuentra en el promedio, sin embargo para los grados II y III, aunque el mayor porcentaje est&aacute; dentro del promedio, es notorio el aumento de pacientes por abajo del promedio; para la grado IV todos los pacientes est&aacute; por debajo del promedio, 45% con retardo mental. (ANOVA F= 110.13 p = 0.000000). RM 2.81 IC 95% 1.07&#150;7.34 <i>p= </i>0.05 (<a href="../img/revistas/gmm/v140n4/a1c2.jpg" target="_blank">cuadros II</a> y <a href="../img/revistas/gmm/v140n4/a1c4.jpg" target="_blank">IV</a>).</font></p>     <p align="justify"><font face="verdana" size="2">De las alteraciones en la audici&oacute;n, 33% se encontr&oacute; en la hemorragia grado IV, aunque no existi&oacute; diferencia estad&iacute;sticamente significativa. La alteraci&oacute;n, en todos los casos fue bilateral y grave. En el lenguaje, la alteraci&oacute;n fue progresiva a mayor grado de hemorragia, (grado IV 78%) aunque no existieron diferencias estad&iacute;sticamente significativas, la alteraci&oacute;n del lenguaje tipo audi&oacute;geno tambi&eacute;n fue mayor para la hemorragia grado IV. La RM no indica aumento de riesgo (<a href="../img/revistas/gmm/v140n4/a1c3.jpg" target="_blank">cuadro III</a> y <a href="../img/revistas/gmm/v140n4/a1c4.jpg" target="_blank">IV</a>).</font></p>     <p align="justify"><font face="verdana" size="2">&nbsp;</font></p>     <p align="justify"><font face="verdana" size="2"><b>Discusi&oacute;n</b></font></p>     <p align="justify"><font face="verdana" size="2">La hemorragia intraventricular es una patolog&iacute;a que influye a corto y largo plazo sobre la evoluci&oacute;n neurol&oacute;gica de los neonatos que la padecen; sin embargo existen otros factores que no se pueden aislar y que contribuyen a la evoluci&oacute;n neurol&oacute;gica, entre &eacute;stos, la prematurez, el bajo peso al nacer, la asfixia perinatal y la enfermedad pulmonar cr&oacute;nica, as&iacute; como la necesidad de ventilaci&oacute;n mec&aacute;nica. Otros autores dan mayor importancia a los d&iacute;as de requerimiento de ox&iacute;geno, as&iacute; como a las alteraciones metab&oacute;licas principalmente la hipoglucemia.<sup>10,28&#150;30</sup> En este estudio no se observaron diferencias significativas en edad gestacional y peso. En la depresi&oacute;n al nacer tomando como par&aacute;metro la evaluaci&oacute;n de Apgar se observa claramente que los pacientes con hemorragia grave (grados III y IV) tuvieron puntajes m&aacute;s bajos al minuto, y ninguna diferencia significativa a los cinco minutos debido probablemente a lo peque&ntilde;o de la muestra en estos grados; sin embargo, para la grado IV los d&iacute;as de requerimiento de ox&iacute;geno fueron mayores, as&iacute; como la presencia de enfermedad pulmonar cr&oacute;nica.</font></p>     <p align="justify"><font face="verdana" size="2">Las lesiones predictivas de futuras secuelas, incluyen hemorragia grado III y IV, ventriculomegalia, ecodensidades periventriculares intraparenquimatosas y leucomalacia periventricular. <sup>16,</sup><sup>22,</sup><sup>31&#150;</sup><sup>33</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">En la valoraci&oacute;n neurol&oacute;gica, como ya lo han referido numerosos autores<sup>5,34,35</sup> se observan predominantemente alteraciones m&iacute;nimas para la hemorragia grado I, aunque un porcentaje peque&ntilde;o puede presentar secuelas graves; al aumentar el grado de hemorragia, las lesiones o secuelas aumentan en gravedad, como se observa en  el presente trabajo, en  la grado  II  la cuadriparesia fue de 6%. Prechtl<sup>33</sup> ha establecido que la espasticidad tiene una relaci&oacute;n marcada con la mala evoluci&oacute;n a largo plazo, y que se puede observar en etapas tempranas antes de los 18 meses de vida al observar la calidad de los movimientos en los lactantes.</font></p>     <p align="justify"><font face="verdana" size="2">Brozynsky<sup>37</sup> relaciona la hemorragia intraventricular con alteraciones en el patr&oacute;n motor; sin embargo &eacute;stas tienden a disminuir con la edad, mientras que las que se observan asociadas a ventilaci&oacute;n prolongada tienden a permanecer iguales.</font></p>     <p align="justify"><font face="verdana" size="2">El da&ntilde;o neurol&oacute;gico grave, incluyendo grados severos de retinopat&iacute;a ha sido asociado con hemorragia grado III o IV y requerimientos de ox&iacute;geno elevados y prolongados, en este estudio 59% del grupo de hemorragia grado III present&oacute; exploraci&oacute;n neurol&oacute;gica anormal, predominando la espasticidad; en la grado IV el 100% tuvo par&aacute;lisis cerebral RM 3.51 IC 95% 1.49&#150;8.28. Se ha sugerido que la par&aacute;lisis cerebral ocurre en los reci&eacute;n nacidos de muy bajo peso y est&aacute; relacionada con la lesi&oacute;n periventricular; sin embargo esta lesi&oacute;n no ocurre en forma aislada.<sup>38&#150;40</sup> La diaplej&iacute;a, se ha asociado con variables antenatales, y la hemiplej&iacute;a, la cuadriplej&iacute;a con complicaciones neonatales y con anormalidades en el ultrasonido.<sup>4142</sup></font></p>     <p align="justify"><font face="verdana" size="2">En la evaluaci&oacute;n de Terman&#150;Menil 56% fue anormal y el cociente intelectual fue disminuyendo a mayor grado de hemorragia; aunque tambi&eacute;n 9% de los pacientes con HIV grado I presentaron alteraciones en las subescalas de razonamiento verbal y num&eacute;rico, 27%, en razonamiento de abstracci&oacute;n visual y 45% con valores por abajo del promedio en memoria a corto plazo, lo que tambi&eacute;n ha sido observado por otros autores<sup>14,21,43</sup> incluso se ha informado asociaci&oacute;n de estas alteraciones con factores sociodemogr&aacute;ficos.<sup>44&#150;46</sup></font></p>     <p align="justify"><font face="verdana" size="2">La enfermedad pulmonar cr&oacute;nica asociada a peso muy bajo al nacer en supervivientes de la UCIN se ha relacionado con pobre desarrollo motor a los tres a&ntilde;os de edad, despu&eacute;s de controlar otros riesgos; tambi&eacute;n se ha observado mayor frecuencia de retardo mental. En el presente estudio la enfermedad pulmonar estuvo presente en todos los grados de hemorragia; sin embargo en la grado IV fue mayor (77%).<sup>47,48</sup></font></p>     <p align="justify"><font face="verdana" size="2">En cuanto a la audici&oacute;n la anormalidad se present&oacute; en todos los grados de hemorragia, la alteraci&oacute;n profunda predomin&oacute; en el grado III y la grave en el grado IV, sin que se encontrara aumento de riesgo cuando se compararon con los grados I/II.</font></p>     <p align="justify"><font face="verdana" size="2">La alteraci&oacute;n en el lenguaje fue desde 25% para la grado I hasta 78% en la grado IV, predominando la alteraci&oacute;n en lenguaje expresivo para los grados I a III, 50% de las alteraciones de la grado IV correspondieron a alteraciones sensorioneurales, Se ha reportado que ni&ntilde;os que persistentemente tuvieron ultrasonido cerebral normal pero eran de peso bajo al nacer tuvieron resultados bajos para la escala de vocabulario (\APPSI); es posible que las lesiones hemorr&aacute;gicas o isqu&eacute;micas invisibles al ultrasonido estuvieran presentes, pero no fueron lo suficientemente importantes para que pudieran ser detectadas; por lo que se ha hipotetizado que sea el resultado de otros factores como la desnutrici&oacute;n, la hipoglucemia subcl&iacute;nica, o la falta de estimulaci&oacute;n temprana.<sup>49</sup></font></p>     <p align="justify"><font face="verdana" size="2">Resnick<sup>44,45</sup> ha asociado significativamente las alteraciones f&iacute;sicas, sensoriales y el retraso mental profundo con factores perinatales (Apgar bajo a los cinco minutos, alteraciones en el ultrasonido, hipoglicemia etc&eacute;tera); los factores sociodemogr&aacute;ficos no parecieron tener efecto sobre las alteraciones con excepci&oacute;n de la raza sobre las secuelas f&iacute;sicas y la educaci&oacute;n materna sobre el retraso mental profundo; sin embargo otros autores,<sup>46,50,51</sup> han asociado las alteraciones del habla y el lenguaje con problemas sociodemogr&aacute;ficos y solamente con un factor perinatal, el transporte neonatal de un centro a otro. Los signos de da&ntilde;o parenquimatoso independientemente de asociaci&oacute;n con hemorragia son indicadores de desarrollo anormal principalmente para secuelas mayores o par&aacute;lisis cerebral.<sup>52&#150;54</sup></font></p>     <p align="justify"><font face="verdana" size="2">Aunque el ultrasonido sigue siendo el mejor instrumento para diagn&oacute;stico de hemorragia por su sensibilidad y f&aacute;cil aplicaci&oacute;n a la cama del enfermo, actualmente la resonancia magn&eacute;tica est&aacute; avanzado en el diagn&oacute;stico m&aacute;s fino de estas lesiones y otras producidas en la sustancia blanca (reducci&oacute;n del volumen, alteraciones qu&iacute;sticas y del cuerpo calloso) o gris (aumento del espacio subaracnoideo) encontr&aacute;ndose como factores de riesgo entre otros la hipotensi&oacute;n, la utilizaci&oacute;n de agentes inotr&oacute;picos, la presencia de infecci&oacute;n perinatal etc&eacute;tera, lo interesante de este procedimiento es la correlaci&oacute;n de las lesiones anat&oacute;micas con la evoluci&oacute;n del neurodesarrollo, sin embargo la limitaci&oacute;n es la de no poder realizar el estudio a la cabecera del paciente.<sup>55</sup></font></p>     <p align="justify"><font face="verdana" size="2">Estudios recientes sobre leucomalacia periventricular (complicaci&oacute;n frecuente de la hemorragia grado IV) han sugerido el posible valor predictivo del electroencefalograma en relaci&oacute;n a la aparici&oacute;n de ciertas ondas agudas con un neurodesarrollo adverso asociado con lesiones de la sustancia blanca causadas por la leucomalacia, incluso estas ondas pueden aparecer una o dos semanas antes de los hallazgos ultrasonogr&aacute;ficos.<sup>56</sup></font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font face="verdana" size="2">En conclusi&oacute;n, el riesgo de presentar alteraciones neurol&oacute;gicas aumenta a medida que el grado de hemorragia es mayor, aunque en las hemorragias grado I y II tambi&eacute;n se presentaron. Fue significativa la observaci&oacute;n de que el cociente intelectual se encuentra por debajo del promedio para toda la muestra; para la hemorragia grado IV 45% de los pacientes presentaron retardo mental.</font></p>     <p align="justify"><font face="verdana" size="2">Aunque la morbilidad neonatal fue parecida, sobresalen la hidrocefalia y la enfermedad cr&oacute;nica pulmonar para los grados III/IV.
