Objetivos:  Evaluar la utilidad del ultrasonido pulmonar (UP) para predecir fracaso a la extubación.  Material y métodos: Estudio prospectivo observacional, de 2013-2015. Inclusión: pacientes &#8805; 18 años, bajo ventilación mecánica invasiva (VMI) &#8805; 24 horas que cumplieron con los criterios de extubación del Consenso Internacional de Medicina Crítica (CEIMI). Exclusión: &#8804; 18 años o aquellos con el fin de limitar el esfuerzo terapéutico. La interrupción de la VMI fue a juicio clínico del equipo médico, de acuerdo a CEIMI. Se realizó en ventilación con presión soporte (VPS), presión soporte (PS) 7 cmH2O, presión positiva continua de la vía aérea (CPAP) 0, Fracción inspirada de Oxígeno (FiO2) 35% durante 30 minutos y UP para evaluar la presencia de líneas pulmonares. Se clasificó en patrones A, B o AB al primer minuto y a los 30 minutos. El destete se consideró exitoso si la respiración espontánea se mantuvo durante &#8805; 48 horas después de la extubación. Grupo A: extubación exitosa y Grupo B: fracaso de la extubación. Análisis estadístico: Recopilación de datos en Excel y análisis estadístico con SPSS versión 16.0. Un valor de p &#8804; 0.05 se consideró significativo.  Resultados:  Se analizaron 80 pacientes. El UP al primer minuto con sensibilidad 81%, especificidad 60%, ABC 0.703, (IC 95%, 0.549-0.857), p = 0.031; UP a los 30 minutos con sensibilidad 81%, especificidad 50%, ABC 0.688 (IC 95%, 0.523-0.852), p = 0.046.  Conclusiones:  El UP al primer minuto es un índice prometedor para predecir el éxito de la extubación. Este nuevo índice formado por las variables cualitativas y cuantitativas necesita ser validado en nuevos estudios.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[Abstract: Despite conducting appropriate spontaneous testing, 10-20% of patients under IMV fail weaning, increasing morbidity and mortality.  Objectives:  Evaluation of lung ultrasound (LUS) to predict extubation failure.  Methods:  Prospective observational study from April 2013 to 2015. Inclusion: patients &#8805; 18 years, &#8805; 24 hours under invasive mechanical ventilation (IMV) that met extubation international consensus criteria (EICI).  Exclusion: &#8804; 18 years or those in order to limit the therapeutic effort. Discontinuation of IMV was decided under clinical judgment of medical team, according to EICI. The setting of the ventilator was in SPV, (PS) 7 cmH2O CPAP 0, FiO2 35% for 30 minutes to assess the presence of lung lines. It was classified into patterns A, B or AB in the first minute and at 30 minutes. Weaning was considered successful if spontaneous breathing was maintained for &#8805; 48 hrs. Successful extubation (Group A), Group B: weaning failure. Statistical analysis: Excel data collection and statistical analysis with SPSS version 16.0. A p value &#8804; 0.05 was considered significant.  Results:  80 patients were analyzed. The LUS in the first minute with a sensitivity 81%, specificity 60%, AUC 0.703 (CI 95%, 0.549-0.857), p = 0.031; LUS 30 minutes with a sensitivity 81%, specificity 50%, AUC 0.688 (CI 95%, 0.523-0.852), p = 0.046.  Conclusions:  LUS at the first minute is a promising index for predicting successful extubation. This new index, consisting of qualitative and quantitative variables, needs to be further validation.]]></p></abstract>

Objetivos:  Avaliar a utilidade do ultrassom pulmonar (UP) para prever a falha da extubação.  Material e metodos:  Estudo prospectivo observacional de abril 2013-2015. Inclusão: Pacientes &#8805; 18 anos, com VMI &#8805; 24 horas que completaram os critérios de extubação do Consenso International de Medicina Crítica (CEIMI). Exclusão: &#8804; 18 anos ou aqueles com limi-tação do esforço terapêutico. A suspenção da VMI foi uma decisão clínica da equipe médica, de acordo com CEIMI. Se realizou com VPS, PS7 cmH2O, CPAP 0, FiO2 de 35% durante 30 minutos e UP para avaliar a presença de linhas pulmonares. Se classificou em padrões A, B ou AB no primeiro minuto e 30 minutos. O desmame foi considerado bem sucedido se a respiração espontânea foi mantida durante &#8805; 48 horas, após a extubação. Grupo A: extubação bem sucedida, e Grupo B: fra-casso na extubação. Análise estatística: Para a recopilacão dos dados se usou Excel, para a análise estatística se utilizou o SPSS, versão 16.0. O valor de p &#8804; 0.05 foi considerado significativo.  Resultados:  Foram analisados 80 pacientes. O UP ao primeiro minuto, com sensibilidade de 81%, especificidade de 60%, ABC 0.703 (IC 95%, 0.549-0.857), p = 0.031; UP aos 30 minutos, com sensibilidade de 81%, especificidade de 50%, ABC 0.688 (IC 95%, 0.523-0.852), p = 0.046.
