</front><body><![CDATA[ <p align="right"><font size="2" face="Verdana"><b>CARTAS AL EDITOR</b></font></p>     <p>&nbsp;</p>     <p><font size="4" face="Verdana"><b>Estudios en M&eacute;xico sobre el gen MTHFR</b></font></p>     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font size="2" face="Verdana"><I>Se&ntilde;or editor: </i>Le&iacute;mos con    atenci&oacute;n la carta de Aguirre&#45;Rodr&iacute;guez <I>et al.</I>, &quot;Prevalencia    del polimorfismo del gen MTHFR en una muestra de poblaci&oacute;n de Nuevo Le&oacute;n,    M&eacute;xico", publicada en el vol. 50 no.1 de <I>Salud P&uacute;blica    de M&eacute;xico</I>.</font></p>     <p><font size="2" face="Verdana">Los autores discuten datos sobre la prevalencia    de la mutaci&oacute;n C677T del gen de la MTHFR en el pa&iacute;s y se refieren    a estudios hechos en M&eacute;xico en pacientes de Nuevo Le&oacute;n, Yucat&aacute;n,    Ciudad de M&eacute;xico, etc., ignorando datos publicados en el pa&iacute;s    con anterioridad. Las primeras publicaciones que hicieron resaltar la alta prevalencia    de la mutaci&oacute;n 677 C&#45;T del gen MTHFR en mestizos mexicanos fueron hechas    en 1999 por Mutchinik <I>et al</I>. en M&eacute;xico<SUP>1</SUP> y por Hessner    <I>et al</I>. en Estados Unidos.<SUP>2</SUP> M&aacute;s tarde, nosotros publicamos    la primera evidencia de la alta prevalencia de esta mutaci&oacute;n<SUP>3</SUP>    en pacientes mexicanos con estados hipercoagulables y luego siguieron los otros    estudios hechos en el pa&iacute;s se&ntilde;alados en la carta de Aguirre&#45;Rodr&iacute;guez    <I>et al</I>. Al haber encontrado la mutaci&oacute;n de este gen en 61% de controles    y en 78% de pacientes con marcadores cl&iacute;nicos de trombofilia, concluimos    que en nuestro pa&iacute;s esta mutaci&oacute;n no se asocia a un estado hipercoagulable,    como otros autores hab&iacute;an sugerido para otras poblaciones. Los datos    que publicamos inicialmente se confirmaron en otro estudio publicado m&aacute;s    tarde con un mayor n&uacute;mero de casos.<SUP>4</SUP> Por otro lado, se debe    considerar con cautela la hiperhomocistinemia como factor de riesgo cardiovascular,    ya que diversos art&iacute;culos recientes han se&ntilde;alado que la normalizaci&oacute;n    de las cifras de homociste&iacute;na no reduce la aparici&oacute;n de eventos    cardiovasculares en mujeres con factores de riesgo.<SUP>5</SUP> Otros trabajos    recientes demuestran que la normalizaci&oacute;n de las concentraciones de homociste&iacute;na    para prevenir la trombosis recurrente no est&aacute; del todo justificada.<SUP>6</SUP>    Un estudio holand&eacute;s<SUP>7</SUP> concluy&oacute; que las concentraciones    elevadas de factor VIII y no la hiperhomocistinemia se relacionan con riesgo    tromb&oacute;tico por lo que en pacientes con hiperhomocistinemia se justificar&iacute;a    investigar las concentraciones del factor VIII:C.</font></p>     <p><font size="2" face="Verdana"> Parece deseable hacer una revisi&oacute;n cuidadosa    de la literatura al referirse a estudios hechos en el pa&iacute;s.</font></p>     <p>&nbsp;</p>     <p align="right"><font size="2" face="Verdana">Dr Guillermo J Ruiz&#45;Delgado,    ]]></body>
