</front><body><![CDATA[ <p align="right"><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>CARTAS    AL EDITOR</b></font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="4"><b>Infecciones    por hongos y neutropenia en un hospital pedi&aacute;trico de tercer nivel</b></font></p>     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Las infecciones    por hongos son un problema emergente en los nosocomios y est&aacute;n asociadas    hasta con 40% de las defunciones relacionadas con procesos s&eacute;pticos.<sup>1-3</sup>    Conocer los principales factores de riesgo que condicionan este tipo de infecciones    ayudar&aacute; al establecimiento de estrategias de control, prevenci&oacute;n,    detecci&oacute;n y tratamiento temprano para beneficio de los pacientes hospitalizados,    principalmente los inmunosuprimidos.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Por ello, analizamos    la frecuencia de los agentes mic&oacute;ticos en pacientes pedi&aacute;tricos    con fiebre y neutropenia, mediante cultivo y la reacci&oacute;n en cadena de    la polimerasa (PCR).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Estudiamos 107    pacientes pedi&aacute;tricos con fiebre mayor de 38 ºC y conteo de neutr&oacute;filos    menor a 500 c&eacute;lulas /&#181;l, con hemocultivo negativo y no respondedores    al tratamiento con antibi&oacute;ticos (cinco d&iacute;as con fiebre persistente).    Se utiliz&oacute; sangre perif&eacute;rica tanto para el cultivo como para la    PCR. Las muestras se incubaron en medio bif&aacute;sico de Ruiz-Casta&ntilde;eda,    y posteriormente en agar dextrosa Saboraud. La identificaci&oacute;n de levaduras    se realiz&oacute; con las t&eacute;cnicas de tinci&oacute;n de Gram y tubo germinativo.    La identificaci&oacute;n de la especie se confirm&oacute; utilizando la bater&iacute;a    de API 20C AUX20 210 (Biomeriux). La identificaci&oacute;n del crecimiento filamentoso    se realiz&oacute; mediante morfolog&iacute;a colonial y observaci&oacute;n al    microscopio utilizando azul de lactofenol.<sup>4</sup></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">El an&aacute;lisis    molecular se realiz&oacute; obteniendo el ADN<sup>5</sup> mediante columnas    de extracci&oacute;n de QIAmp Qiagen Tissue Kit&reg; de acuerdo con las indicaciones    del fabricante. La PCR se realiz&oacute; utilizando los iniciadores que reconocen    la regi&oacute;n conservada de la subunidad 18S del RNA.<sup>6</sup> Para la    identificaci&oacute;n, se utilizaron seis sondas, marcadas con biotina espec&iacute;ficas    para <i>C. albicans</i>, <i>C. tropicalis</i>, <i>C. krusei</i>, <i>C. glabrata,    C. parasilopsis</i> y <i>Aspergillus sp</i>.<sup>7-9</sup></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">De acuerdo con    el diagn&oacute;stico, los 107 pacientes se clasificaron en cinco grupos: 1)    oncol&oacute;gicos, 2) con malignidades hematol&oacute;gicas, 3) los sometidos    a trasplante, 4) con hipogamaglobulinemia y 5) con falla hep&aacute;tica fulminante.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Encontramos que    39% fueron mujeres y 61% hombres entre 2 y 14 a&ntilde;os de edad. La mayor&iacute;a    de los casos pertenecieron al grupo oncol&oacute;gico, principalmente ni&ntilde;os    con leucemia linfobl&aacute;stica aguda (21 muestras), seguido del grupo de    trasplantes, especialmente los de m&eacute;dula &oacute;sea (14 muestras).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">De 107 pacientes,    13 fueron positivos a hongos por cultivo (12.1%) y 49 (45.7%) por PCR, diferencia    estad&iacute;sticamente significativa (<i>p</i>&lt;0.05). La frecuencia de especies    encontrada fue: <i>Candida albicans</i> (17.8%), <i>Aspergillus sp</i> (9.3%),    <i>C</i>. <i>glabrata</i> (5.6%), <i>C. tropicalis</i> (3.7%), <i>C. krusei</i>    (0.9%), <i>C. parasilopsis</i> (0.9%), <i>Candida sp</i> (3.7%), <i>Fusarium    sp</i> (0.9%), <i>Sepedonium sp</i> (0.9%), e <i>Histoplasma sp</i> (0.9%).    Adicionalmente a la frecuencia, si consideramos al PCR como el est&aacute;ndar    de oro, entonces la sensibilidad del cultivo fue de 26.5% (13/49), la especificidad    de 100% (58/58), el valor predictivo positivo (VPP) de 100% (13/13) y el valor    predictivo negativo (VPN) de 38.3% (36/94). No hubo muestras con cultivo positivo    y PCR negativo.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>Candida albicans</i>    seguida por <i>Aspergillus sp</i> siguen siendo las especies m&aacute;s frecuentemente    asociadas con la presencia de neutropenia y fiebre en pacientes inmunocomprometidos.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Logramos identificar    especies que no se reportan frecuentemente y que son emergentes y de importancia    cl&iacute;nica entre las que destacan <i>C. glabrata</i>, <i>C. krusei</i> y    <i>Fusarium sp</i>, caracterizadas por su resistencia a los antimic&oacute;ticos    de uso com&uacute;n como la anfotericina B y fluconazol.<sup>10,11</sup></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Ya que observamos    una baja sensibilidad del cultivo con respecto al de PCR, con una tasa de falsos    negativos de 73.5% (36/49), es decir, de cada 100 personas con infecciones oportunistas    por hongos, 74 de ellas no est&aacute;n siendo detectados y tratados adecuadamente,    es por ello que sugerimos que la PCR se incorpore al an&aacute;lisis rutinario.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Este trabajo fue    aprobado por el comit&eacute; de Investigaci&oacute;n del Hospital Infantil    de M&eacute;xico Federico G&oacute;mez, registro HIM/98/019.</font></p>     <p>&nbsp;</p>     <p align="right"><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Dr    Jos&eacute; Arellano-Galindo,    <br>   M en C M&oacute;nica Moreno-Galv&aacute;n,    <br>   Hospital Infantil de M&eacute;xico Federico G&oacute;mez.</font></p>     ]]></body>
