Evaluation of bacterian microleakage in extracted teeth using amalgam, Super EBA, MTA and Portland cement as retrofilling materials

Resumen

La cirugía periapical es un procedimiento que se realiza en la presencia de patosis periapical, cuando el tratamiento endodóntico de rutina o el retratamiento han fracasado. La mayoría de los fracasos en el tratamiento de conductos se debe a una contaminación con microorganismos, siendo el Enterococcus faecalis el principal responsable del mismo. El propósito de este estudio fue medir la microfiltración bacteriana (Enterococcus faecalis) en dientes extraídos utilizando diferentes materiales de retroobturación como: amalgama, súper EBA, MTA, y cemento Portland.

Se utilizaron 63 dientes extraídos unirradiculares, instrumentados con la técnica de fuerzas balanceadas, irrigados con NaOCl al 5.25%, REDTA, y gluconato de clorhexidina al 0.12%.

Se formaron 4 grupos experimentales de 15 dientes cada uno y 3 grupos control de 1 diente cada uno a los cuales se les realizaron apicectomías, y retropreparaciones ultrasónicas. Los grupos experimentales se retroobturaron con amalgama, súper EBA, MTA y cemento Portland.

Los dientes se inocularon con una suspensión de Enterococcus faecalis y la porción apical se puso en contacto con el medio de cultivo durante 15 días.

La microfiltración bacteriana se evaluó en los diferentes grupos, encontrándose el mismo grado de microfiltración para el MTA y sEBA, seguidos por el cemento Portland y la amalgama.

Abstract

The periapical surgery is defined as the procedure that is made in presence of periapical pathosis when the common endodontic treatment or retreatment has failed. Most of all failed cases in endodontic therapy are caused by microbiological occupation of the root canal space. It has been demonstrated that the main species responsible of failure in root canal therapy is the Enterococcus faecalis. The purpose of this study was to measure Enterococcus faecalis microleakage in extracted teeth using different retrofilling materials: amalgam, Super EBA, MTA and Portland cement. For this study sixty three single rooted extracted teeth were used. They were instrumented with the Balanced Force technique, irrigated with NaOCl 5.25%, REDTA, and Clorhexidine Gluconate 0.12%. Four experimental groups of fifteen teeth and three control groups of one tooth each were randomly formed. In each teeth apicoectomy was done and ultrasonically retropreparations were made. Experimental teeth were retrofilled with amalgam, Super EBA, Portland cement and MTA. In all teeth a suspension of Enterococcus faecalis was inoculated, and the apical portion of these teeth remained in contact with the medium for 15 days. Microleakage was evaluated in the different groups, showing that the MTA and Super EBA groups had the lowest microleakage, followed by the Portland cement and amalgam groups.
