Buerger's disease (thromboangiitis obliterans) is a rare entity, commonly affecting young men (<45 years) and always associated with smoking. This affection can get to compromise the extremity, being cause of major amputations. Today, it continues to be a disease of unknown etiology; however, its pathogenesis is attributed to various mechanisms, especially autoimmune, proinflammatory and hypercoagulable states. It involves the affection of the distal vessels of the extremities, through a non-atherosclerotic process, with inflammatory and thrombotic pathophysiology, always associated with smoking as the main etiopathogenic factor. Endovascular therapy with percutaneous transluminal angioplasty has recently boomed as a valid therapeutic option in patients with Buerger. In some series, favorable results are evident, with a 12-month success rate of up to 86.7% in limb salvage rate (LSR) and 66.7% in improvement of symptoms with a low rate of complications, so today it is a safe and effective procedure for the preservation of the extremities.
