Introduction:

Fungal keratitis are aggressive entities, with sequelae that can lead to permanent visual impairment.

Objective:

We describe a case of corneal ulcer caused by an infrequent agent, Fonsecae pedrosoi, highlighting the difficult medical and surgical treatment that finally allowed anatomical and functional preservation of the eyeball.

Case report:

A young male, whom after an ocular trauma with vegetable material, presents a fungal corneal ulcer, isolating a dematiaceous fungus identified as F. pedrosoi in the cultures. The patient received prolonged antifungal management and crosslinking; however, he had multiple relapses with a corneal microperforation that required treatment with a cyanoacrylate patch on two occasions, and finally, a penetrating keratoplasty.

Conclusions:

Fungal corneal ulcers are relatively frequent in rural areas in Latin America. It is the ophthalmologist’s job to diagnose them early and treat them in a timely manner, but also its prevention by educating the population at risk.
