Gastrointestinal stromal tumors (GISTs) that develop outside the digestive tract are called extra-GISTs (eGISTs), eGISTs account for approximately 5-10% of all GISTs, Ovarian eGISTs are rare, Most GISTs harbor an activating kinase mutation in KIT or PDGFRA. For eGISTs, the incidence of this type of mutation is 40–50%. eGISTs may have a worse prognosis compared to conventional GISTs with high mitotic rates, large size and distant metastases, including lymph node involvement, the second case in the world reported by ovarian eGIST is presented.
