The design of effective chemoradiation therapies for cancer patients relies on finding the balance between normal tissue toxicities and a therapeutic dose. Treatments for diseases in regions with a large reservoir of active bone marrow can impede the ability to deliver a full chemotherapy and radiation therapy regimen. There are many image techniques that have allowed the identification of hematopoietically active regions of bone marrow that could be spared with advanced radiotherapy techniques such as intensity modulated radiotherapy. Limiting the radiation dose to proliferating bone marrow might reduce the severity of hematologic toxicity, improve tolerance to radiation fields, chemotherapy, prevent hospitalization and decrease the need for transfusions or growth factors. The objetive of this review is to define bone marrow sparing radiotherapy techniques that may be applicable to diseases treated with radiotherapy and chemotherapy in order to improve cost benefit to patients.
