Severe burns are associated with systemic coagulopathy that resembles those found in patients with sepsis or major trauma. It is characterized by procoagulant-anticoagulant imbalance and hyperactivity of the fibrinolytic system. It is secondary to hemodilution, hypothermia, inflammation, hypoperfusion and endothelial injury. Burn coagulopathy is associated with increased morbidity and mortality. Even when recognized, there are no guidelines for the specific management of coagulopathy in burns, but supportive measures and targeted treatments, like in trauma coagulopathy, have been proposed.
