Acute appendicitis management represents a therapeutic challenge for every surgeon because of the marked increased complication rate due to tissue fragility. In this article we present the case of a patient who was admitted to the emergency room with complicated acute appendicitis. He had two intraabdominal abscesses with volumes of 88 and 125 ml in right lower quadrant and in hypogastrium. Conservative management was indicated with ultrasound-guided aspiration and drain of abscesses, broad spectrum intravenous antibiotics and parenteral feeding. The patient had a positive response to treatment and was discharged after nine days. We also present a review of the current literature regarding complicated acute appendicitis management.
