Surgical simulators do not represent reality faithfully due to technological and design limitations. Fidelity depends on the haptic quality of the simulator. The problem is whether they can be a cognitive instrument or if their role is limited to the acquisition of manual skills. If, despite the simulators, the National Surgical Board exam failure rate is about 40%, the problem is elsewhere: in the careful monitoring of residents over the years of training. Simulators cannot replace a supervised training, one-on-one.
