Resumen

Objective: To evaluate the usefulness and effectiveness of the diagnostic scale Alvarado, laboratory and imaging tests in acute appendicitis underwent a series of clinical cases surgically

Material and methods: Clinical records of 152 patients who underwent surgery with a diagnosis of acute appendicitis September 2012 to September 2013 according to the diagnostic scale Alvarado and correlated with histopathology, duration, sex and age were reviewed.

Results: Of the 152 files reviewed, 88 patients were male and 64 female, mean age was 27.8 years, time of onset of symptoms before admission 37.5 hours. The most common symptom was pain in the right lower quadrant in 98.6%, Blumberg 71%, 50.6% migration of pain, nausea or vomiting 75.6% , 39.4% anorexia, temperature above 37.2 °C 42.1% , 92.7% leukocytosis, 90.7% neutrophilia. The 7.89% of the appendectomies were negative, appendice infl ammation 47.9%, 11.8% gangrenous, perforated with localized peritonitis 19%, and perforated with generalized peritonitis 13.1%. No patient was reported histopathologically with a tumor, or parasites. Ultrasound was used in 11.1% of cases and Computed Tomography in the 1.9%.

Conclusion: The diagnosis of appendicitis is primarily clinical and is a challenge for the primary care physician and the surgeon found in units that do not have imaging studies.
