BACKGROUND:

The transverse vaginal septum is a rare failure of vaginal embryogenesis. It has been associated with infertility, obstetric complications and recurrence. Cases of vaginal delivery after surgery are scarce.

OBJECTIVE:

To describe a case of gestation in the early postoperative period which culminates in vaginal delivery after vaginal dilation during pregnancy.

CLINICAL CASE:

A transverse vaginal septum was incidentally found in the patient during the diagnosis of an abortion. An aspiration curettage was carried out through the fenestration of the septum, performing a double-cross plasty 12 weeks later. Surgery was scheduled after diagnostic confirmation by ultrasound and Magnetic Resonance Imaging. In the early postoperative period, the patient got pregnant again, carrying out a close follow-up with training in vaginal dilatation. Gestation culminated in a vaginal delivery without subsequent recurrence.

CONCLUSION:

Scheduled double-cross plasty with constant vaginal dilation, along with the gestation itself, could diminish the complications associated to the transverse vaginal septum.
