Purpose: To determine the efficacy of bronchial artery embolization in the control of massive hemoptysis. Material and methods: From January 1997 to December 2003, 30 bronchial artery embolizations were done in 24 patients (10 women and 20 men), ages from 2 years to 77 years old, mean age 47 years. Most cases were due to tuberculosis. The way of access was the right femoral artery and the bronchial arteries were occluded with polyvinyl alcohol microspheres with or without the combination with vascular coils. Results: Bronchial hipervasculature, contrast medium leakage, bronchial-pulmonary artery shortcut and dilatation of bronchial artery embolization were seen in 82.3% of the cases during the first week and 66.6% after 6 months follow-up. Three patients needed multiple embolization and 6 patients needed surgery to control bleeding. Conclusion: Bronchial artery embolization is an effective method for the control of moderate to massive hemoptysis. Most of our cases were due to pulmonary tuberculosis followed by bronchial-pulmonary artery shortcut. Better results are obtained with a combination of polyvinyl alcohol microspheres and vascular coils.
