In previous reports in this institution wass demonstrated that brain tuberculomas is curable with medical treatment. The diagnose can be made without evidence of extracranial tuberculosis. The following ones were clinics characteristic of tuberculoma: reinforcing of the tuberculoma in TC, avascular injuries in the angiography, the signs and symptoms are minors who happend in relation to the size and its location, the patient lives in an endemic area or migrate of this. No exist findings of neuroimaging differentiates tuberculoma from another intracranial injury. Exists greate number of false positives for tuberculomas when they are diagnosed through Computerized Tomography. A report indicates that 80% of the diagnosed cases as tuberculoma by TC were false positives. The brain tuberculoma is very rare and its location in the sinus cavernous is exceptional many factors make difficult the diagnose, sometimes is necessary a surgical procedure. We presente a case of a woman of 46 years of age with right ocular pain and diplopía, the examination reveal a syndrome of the cavernous sinus, the radiological investigation demonstrated an injury at level of the cavernous sinus considering the diagnose of meningioma, Sarcoidosis, metástases or tuberculoma but no of these was confirmed. The PCR study (chain reaction of the polymerase), in the cerebrospinal fluid was positive for Tuberculosis. Beginning the antifimic treatment a resolution of its symptoms was archived.
