Ultrasound has been positioned as an excellent tool for the anatomical and functional evaluation of the diaphragm in the critically ill patient. It is a simple, portable, no-risk imaging method, and it is cost-effective. Two ways of measuring the diaphragmatic function have been proposed and validated in multiple studies. The first one evaluates the mobility of the diaphragm with the M-mode ultrasound and is known as diaphragmatic excursion; the second is the measurement of changes in diaphragm thickness during a respiratory cycle, known as delta diaphragm thickness (Δtdi). The purpose of this review is to present the functionality and importance of both assessments in the critical patient.
