To present the case of a patient with episodes of syncope and hypotension related to the use of latanoprost. This is the case of a 32-year-old man with a bilateral enlarged cup/disc ratio, normal visual fields, an abnormal optic nerve as seen on the optical oherence tomography, and a 582/563 μm pachymetry. Eye examination: best-corrected visual acuity of 20/20 in both eyes, IOP, 13/13 mmHg OU, and a cup-to-disc ratio of 0.7/0.7. A water drinking test was performed with a IOP peaks of 22 mmHg in OD, and 21 mmHg in OS. Latanoprost was prescribed to both eyes. Afterwards, he had 3 episodes of syncope, with arterial hypotension that required hospital care. No associated structural or functional cardiovascular lesions were found. At this point, the possibility of a secondary effect due to the use of latanoprost was considered. When episodes of hypotension or syncope are documented in a patient on latanoprost and no other possible triggers are found, it seems prudent to choose another pharmacological or non-pharmacological hypotensive treatment for the management of glaucoma control.
