The present work proposes lean mass preservation as a central component of the Intensive Care Unit. Protein-energy malnutrition is the most prevalent form of malnutrition among critically ill patients. This compromises the patients’ functional capacity and their immune system, increasing recovery time and mortality rates. Protein anabolism relies both on adequate protein intake and muscle stimulation. Until now, nutritional intervention protocols for critically ill patients are independent from physical therapy and medical treatment. The present work proposes a protocol towards lean mass preservation in the critically ill, which integrates the roles of the essential team work, including critical care specialist, nutritionist, nurse and physical therapist. This protocol integrates interventions that have proven to be effective in decreasing morbidity, mortality, days of hospital stay, days on ventilatory support, and hospital readmissions. The proposed tools for diagnosis and monitoring of nutritional and functional parameters have been validated in previous studies.
