The term diabetic cystopathy is attributed to the deterioration of detrusor function caused by a combination of myogenic and neurogenic mechanisms, resulting in an increase in postmicturition residual bladder volume capacity and alteration, accompanied by reduced bladder contraction and sensation, in diabetic patients. The disease primarily originates from sensory and autonomic neuropathy due to alterations in the bladder nerve fibers. Clinical presentation is insidious, and when a symptom, usually urinary incontinence, does appear, the disease is in an advanced stage. Diagnosis is made through clinical presentation, associated with neurologic symptomatology, and confirmed through urodynamic studies. Strict control of diabetes mellitus, with treatment adherence, is essential for preventing complications in the long term, such as diabetic cystopathy. Once the disease is diagnosed, treatment can be pharmacologic, with a wide variety of drugs to choose from, and/or surgical, such as bladder denervation or myomectomy.
