Tuberculous lymphadenitis is usually indistinguishable from metastatic forms of papillary thyroid carcinoma (PTC). A 31-year-old man assessed for cervical lymphadenopathies with a TI-RADS (Thyroid Imaging Reporting and Data System) 5 ultrasound report of a thyroid nodule, with positive fine-needle aspiration biopsy for PTC. Thyroidectomy with lymph node resection was performed with report of lymph node with a result negative for metastasis, positive Ziehl-Neelsen. Antituberculous management is started. The typical presentation of cervical tuberculous lymphadenitis has features in common with metastatic PTC nodes. Cervical lymphadenopathy in a patient with a PTC does not always indicate metastatic spread of the disease.
