Purpose:

To report the management of a patient with primary cornea guttata symptoms after cataract surgery with a trifocal intraocular lens (IOL) implant.

Case report:

A 67-year-old male was referred to our department due to persistent blurred vision and night glare 6 months after phacoemulsification and trifocal IOL implantation. By slit-lamp examination, corneal guttae was observed and changes in corneal endothelium morphology suggested primary cornea guttata. A monofocal IOL exchange was performed in order to reduce corneal aberrations, with an excellent outcome after 2 years of follow up.

Conclusions:

Corneal guttae accent optical aberrations in combination with premium IOLs. Screening for endothelial dysfunction is a key element for successful outcomes in cataract surgery.
