Histoplasmosis is the most common mycosis in the world, in Mexico it is found throughout the national territory. It can have acute and chronic presentations. This is the case of a 50-year-old woman with a respiratory clinical picture of two months of evolution, who presented with a pericardial effusion with hemodynamic compromise, was scheduled for surgery finding purulent pericarditis, draining peurulent effusion and performing partial pericardiectomy, with cultures negative and positive urine and serum studies for H. capsulatum. He presented adequate evolution. It should be borne in mind that a rare cause of culture-negative purulent pericarditis may be histoplasmosis, especially in long-standing clinical conditions, such as residual forms of histoplasmosis.
