Background:

Atherosclerotic occlusive disease affecting the brachiocephalic trunk is rare.

Methods:

A 59-year-old patient who attended the emergency department with weakness in the upper left limb. On physical examination, blood pressure asymmetry between the upper limbs and absence of pulse in the right upper limb were evident. The tomography revealed calcified atheromatosis in both internal carotids and 8 in length was observed at the origin of the TBC, greater than 90%, with post-stenotic dilatation. Using a femoral puncture and cervicotomy, an 11 x 29 mm balloon-expandable covered stent was implanted.

Results:

In the review at 6 months, the absence of new neurological focus was evident, and with images in angiotomography the permeability of the stent.

Conclusions:

The choice of the VIABHAN VBX in this case gave us the reliability of overcoming a punctual, ostial lesion and adjusting it to post-stenotic dilation.
