Purpose:

To determine the utility of the neutrophil-to-lymphocyte ratio as a predictor of rejection in patients treated with penetrating keratoplasty

Methods:

Retrospective, observational, analytical study. The Kruskal-Wallis test, chi-square test, ROC curve, and area under the curve were used.

Results:

A total of 111 out of the156 patients included did not show rejection (Group A) while 45 did show endothelial rejection (Group B). Keratoconus was present in 47.74% (group A) and bullous pseudophakic keratopathy (group B) in 35.55% (p = 0.029). The mean time to present rejection was 9 ± 7 months. The greatest risk factor was glaucoma (26.66%). The mean neutrophil-to-lymphocyte ratio of group A was 1.94 ± 0.67, and of group B, 2.49 ± 1.46 (p = 0.032) The ratio cut-off value to present rejection was 2.04.

Conclusion:

The mean preoperative visual acuity in both groups was similar while the postoperative visual capacity in group A was lower. Glaucoma was the greatest risk factor. There was a significant difference in the ratio of group B. The ratio cut-off value to predict corneal rejection was significant (≥ 2.04).
